Community Care, voted one of the Best Places to Work in Maine, is seeking a physician's assistant or certified nurse practitioner to provide consultation to our Behavioral Health Home (BHH) Program. This is a contracted position and would require approximately 4 hours per month. The role of the consultant is to collaborate with BHH providers/team members to implement evidence-based practices in working toward increased population health for adult consumers in an integrated physical and mental health model. This position does not require individual client consultation.
Minimum Qualifications: Must be a physician's assistant licensed as such by the State of Maine, or a certified nurse practitioner who meets all of the requirements of the licensing authority of the State of Maine.
