Job Description

Community Care , voted one of the Best Places to Work in Maine, is seeking a Full Time Targeted Case Manager to provide case management to children diagnosed with mental/behavioral health needs, assisting families in accessing natural and formal supports to help the child in reaching his/her treatment goals. The TCM will also provide services to women who are parenting young children. Case Management involves identification, assessment, planning, linking, monitoring, and evaluation of services and supports for youth to promote successful continuity of services, minimize the need for hospitalization, and support the client's current living status to reduce cycles of instability and enhance the quality of life.

Minimum Qualifications: Bachelor's degree in a related field and child case management experience
